The California Public Records Act provides public access to certain records maintained by the County of Sonoma.

The form may be used to request public records from the Sonoma County Department of Health Services. The California Public Records Act (CPRA) provides public access to certain records maintained by the County of Sonoma. When a person makes a public records request, regardless of whether it is oral or written, the County shall respond in accordance with the provisions of the California Public Records Act.
